AFTERCARE

OVERVIEW

Aftercare at DHI focuses on assisting clients to develop a satisfying, balanced and meaningful life. The aftercare co-ordinator works on a 1-1 basis with clients to assist them to identify their own long and short term lifestyle goals, which may include employment, voluntary work, training, education, skills development, health, social life and to provide and access support and funding.

JOBS, EDUCATION AND TRAINING

We offer support with accessing and funding college courses. Access to some free or reduced costs college courses and community learning courses. Links with recruiters for employment opportunities. Advice information and support with benefits, accessing grants and letter writing.

RELAPSE PREVENTION GROUP

The relapse prevention group is for clients who are maintaining changes to previous drug taking behaviour or in shared care drug treatment. The groups run for 6 weeks and cover topics such as Triggers, Cravings, dealing with lapse and setting up an action plan. “Big Urself Up” is a 6 week course which looks at how to bring about positive changes and increase self esteem. The course covers topics such as goal setting, managing stress, social support and assertiveness skills.

SOCIAL ACTIVITIES

We can offer support with starting new activities, e.g. voluntary work, access to leisure centres and other sports activities, links with the Boxing Academy, as well as information, advice and guidance with funding activities.

INFORMATION SURGERIES

Offer clients the opportunity to find out about services they may want to access within Swindon. Info surgeries have included attendance by Sexual Health Clinic, Swindon Volunteers and Artline. Clients do not have to be involved in any form of treatment to access the surgeries; they can simply drop in.

ELIGIBILITY/REFERRAL CRITERIA

People aged 18 and above, living within Swindon who would like information, support and advice around their drug use and/or would like to access treatment services. You can self refer or be referred by your drugs worker.

AVAILABILITY/OPENING HOURS

The service is available 9.00 am – 6.00 pm Monday to Friday and can be accessed in the following ways:-

Drop in @ Maxwell Street – 11 am – 2 pm Monday to Friday

Groups and 1 to 1s are available by appointment @ Maxwell Street between 10am – 2pm Monday to Friday and if the above times are not accessible, by arrangement outside those hours at other venues, a telephone appointment or home visit.
